New Patients

If you have never visited our offices, please create a new patient account to start using our patient portal.  Once you have your account, you can request an appointment or complete the information necessary for your appointment.  If you need help with your account, please feel free to call our office at (323) 954-1788.
 

Current Patients

If you are currently a patient of our practice, you can now use this patient portal for requesting a new appointment or prescription refill, communicate with our office, update your records, view your lab results, view your statments and pay your bills online.  If this is your first time using our patient portal, please click "Forgot your password" under the "Current Patient/Family Login" section. If you need help with your account, please feel free to call our office at (323) 954-1788. If this is a true medical emergency, please call 911.

Notice of Access to Health Information:

We support our patients having access to their health information through the Patient Portal. Beyond the Patient Portal, we can enable certain mobile apps that meet requisite technical specification and security requirements to provide you access to your health information in a safe and secure way. Currently no mobile apps have been configured to connect to our system. Our portal will be updated if any additional apps become available for you to use to access your health information.
